We coordinate directly with care agencies and case managers to ensure continuity of care and compliance with program requirements.
CalAIM & Enhanced Care Management (ECM)
Divine Light Care Home is an official CalAIM community partner, collaborating with Enhanced Care Management (ECM) providers and care coordination agencies throughout San Diego County. Through CalAIM, we support individuals with complex medical and social needs by providing safe residential placement, structured supervision, and coordinated care services.

Meet the Owners
Divine Light Care Home is owned and operated by Rogelio (RJ) Diaz and Catherine Diaz. As hands-on owners and certified California RCFE Administrators, we are directly involved in daily operations to ensure the highest standards of care, safety, and regulatory compliance.
Rogelio (RJ Diaz holds a Bachelor's degree in Health Care Administration and brings extensive leadership experience as a Retired U.S. Navy Logistics Specialist Chief. His background in healthcare administration, operational management, and prior caregiving experience supports strong organizational structure, accountability, and regulatory compliance within the home.
Catherine Diaz is a Licensed Vocational Nurse (LVN) with clinical experience in resident care, medication management, and care coordination. Her nursing background provides structured oversight, enhanced clinical communication, and support for individualized care planning.
Together, we combine clinical expertise, state-certified administrative leadership, disciplined management, and compassionate caregiving to provide a safe, well-managed residential environment for every resident entrusted to our care.
We actively collaborate with families, healthcare providers, ALWP case managers, CaAIM partners, and San Diego PACE teams to ensure seamless care coordination.
